The Georgia Association of the Deaf Awards Special Award Nomination Form -> download form.   Awards will only be presented to those who work with the Deaf, Hard of Hearing, and/or Deaf-Blind community.   Person nominated for an award can only be Deaf, Hard of Hearing, or Deaf-Blind.   Nominate someone who deserves an Award!!   Nomination Requirements: 1) Nomination form with detailed explanation 2) Company logo or a picture of the person nominated 3) The nominated recipient's contact information 4) Your name must be printed and signed - you only may nominate once for each award 5) Submit then wait and see if your reipient will win the nomination on May 1st, 2007. * Only the top 4 nominees for each award will only be announced *
