| At each GAD conference, the GAD gives 4 awards to qualified and deserving recipients. We are seeking nominations for the following awards. If you would like to nominate someone for an award, please write on a sheet of paper or print form and send to the GAD by June 1, 2007. |

There are many people who have contributed to the Deaf Community and we want to honor them by giving them something to remember that their contributions are recognized and remembered. |

| Awards are for: |
| 1. Outstanding Georgia Organization |
| 2. Outstanding Young Deaf Citizen |
| 3. Outstanding Senior Deaf Citizen |
| 4. Outstanding GAD Supporter |

| Please print out (or write on the sheet) the form and explain in 3 - 5 sentences the person's past or present contributions/performances and why you feel this person deserves this award. |
